Consent & Release Form
Purpose. This release applies to photographic, audio or video recordings, testimonials, and course material collected as part of my participation in the In Motion & Momentum+ program. By signing this release I understand that photographic, video recordings of me, or testimonials and course materials written or spoken by me may be used by the Canadian Career Development Foundation (“CCDF”) and/or its affiliates in the promotion of its products and services.
Legal Release. I hereby grant, in perpetuity without geographic restriction, to CCDF and/or its affiliates the right to display, publicly perform, exhibit, transmit, broadcast, reproduce, record, photograph, digitize, modify, alter, edit, adapt, or otherwise use my name, image, likeness, appearance, voice, written and spoken testimonials and course materials, in any medium or format, on any platform (the “Materials”) for the purpose of research, advertising, public relations, publicity, and promotion of CCDF’s products and services without further consent from or royalty, payment, or any other consideration to me. I acknowledge and agree that I have no right to review or approve any of the Materials before they are used by CCDF or its affiliates.
Intellectual Property Assignment and Waiver. I agree that CCDF shall be the exclusive owner of all rights, title, and interest in and to the Materials, including all copyright. I hereby irrevocably transfer, assign, and otherwise convey to CCDF my entire right, title, and interest, if any, in and to the Materials and all copyright and other intellectual property rights therein. I irrevocably waive all so-called “moral rights” that I may have to any Materials.
CCDF Use in Good Taste. CCDF shall not use the Materials in bad taste or for purposes other than stated herein. Non-public Materials will be stored in a secure location as long as they are relevant and after that time destroyed or archived and will only be accessible to authorized staff.
Revoking of Consent. I understand that I may revoke my consent at any time by notifying CCDF in writing by emailing information@ccdf.ca. This revocation will not affect any actions taken by CCDF or its affiliates before receipt of this written notification.
Release. By signing this form, I acknowledge that I have completely read and fully understand the above release and agree to be bound thereby. I hereby release CCDF, its employees and affiliates, from any and all claims relating to the use of the Materials, including but not limited to, rights of personality, intellectual property, publicity, and invasion of privacy.
If this release is obtained from a person under the age of 18, then the signature of the parent or legal guardian is also required.
